Trinity Care Center (TCC) is a state licensed, non-profit adult day care and activities program.  For the past eleven years is has been providing supervised care on a daily basis to adults who are unable to stay home alone. TCC, being wheelchair accessible, has been able to provide its services to the individuals who are wheelchair-bound. It also provides an opportunity for those who are socially isolated, to socialize and make new friends.

 

·      TCC serves the area of Bedford, Coffee, Franklin, and Moore Counties. 

·      We are an agency of the United Way of Highland Rim and United Way of Franklin County.

·      Our staff includes a full-time director and two program assistants.  One volunteer registered nurse is available for consultation and other needs.

 

 

Our Goals Include:

·      To provide a safe, supportive and caring environment to the participants.

·      To offer a cost-efficient way of providing long-term care.

·      To provide respite and prevent caregiver burnout. This allows families to continue caring for their loved one in their home for a longer period of time.

·      To provide an opportunity for socialization to the participants who are socially isolated through the group setting and activities of our program

·      To provide respite, support, and education for the families and other caregivers on an on-going basis to meet their ever changing needs. This also allows caregiver the option to continue with their present employment.

·      To increase the self-esteem of the participants through activities specifically tailored to meet their needs, using their abilities and strengths.

·      To serve as a liaison between families, physicians, and other community services.

 

Participants:

·      Provides a safe, supportive and friendly place with supervised care by loving professionals.

·      Our group setting provides plenty of opportunity to socialize with a group of people.  This helps the individuals who are socially isolated and are lonely.

·      The activities are geared towards helping the participants feel successful, which gives them a sense of accomplishment.

·      The sense of accomplishment increases the participants’ self-esteem.

·      Participating in various games and activities, while making new friends increases the enjoyment of each day.

·      Physical activities and games along with a regular exercise program helps the participant to maintain or improve the physical functioning by improving circulation, joint flexibility, and muscle strength.

·      Mental activities and games exercise their minds, which helps maintain or improve the participants mental functioning.

Caregivers:

·      Our program provides much needed respite for the caregiver.

·      Receiving respite helps prevent caregiver burnout, which is one of the major problems in providing continued long-term care at home.

·      This helps the caregiver keep their loved one in the community for a longer period of time.

·      Respite gives the caregiver time to meet many of their everyday needs, including the healthcare needs.  It is a fact that caregivers who take care of themselves can take better care of their loved ones.

·      The staff works as a liaison between the participant, the caregiver, and the healthcare providers to meet the ever-changing need of the participant and the caregiver.

·      The staff also provides education and support to the caregivers to meet the challenges of providing long-term care at home. 

·      The staff also helps support the caregiver in planning the future care of the participant. Our staff makes referrals to other services in the community when necessary.

 

 

 

The following criteria take into consideration the staff size, training and safety of the participants in our group setting.

 

·               The applicant is required to have an initial screening/assessment interview by the staff of TCC in the presence of the family member/caregiver.

·               The applicant needs to be able to understand and follow simple instructions.

·               The applicant needs to be able to fit into a group setting. Individuals requiring constant supervision will not be eligible.

·               The family member/caregiver will be expected to provide the name and telephone number of a person that can be reached in case of an emergency.

·               The applicant must be continent of bowel and bladder. He/she needs to be able to attend to bathroom needs independently or with minimal assistance from the staff. If needed, reminders will be given and a staff member will guide the individual to the bathroom.

·               The applicant should be able to feed himself/herself, with minimal help.

·               The applicant must not require nursing care, since the staff is not qualified to provide this type of care.

·               If the applicant is going to require medication while at the TCC, the caregiver/family member will need to bring the medication in a clearly marked container with the name of the medication and the person, the dosage and the schedule. The medication will be stored in a locked cabinet at TCC. The staff will not administer the medication, but will assist the person with it. No shots will be given, since the staff is not trained to do so.

·               The applicant with aggressive or violent behavior, or with the history of such behavior will not be eligible. Once enrolled in the program if such behavior surfaces later on, the individual will be immediately discharged from the program at the discretion of the staff. Safety of each and every one is the top most priority.

·               Once enrolled in the program, family members/caregivers will be responsible for providing transportation.

·               After enrolling into the program, the caregiver/family member will need to provide at least one complete set of change of clothing for the participant to be kept at TCC.

·               Participant will not be allowed to attend TCC while they have a contagious illness or fever, including the common cold.

 

 

Our staff has loving individuals who provide the opportunity for each participant to feel accepted and nurtured on a daily basis. We realize that majority of the individuals we serve commonly suffer from multiple losses like losing loved ones, friends, physical health, financial security, employment, independence, etc. Some of them are going through an adjustment and/or grieving process.  Keeping all these things in mind, our activities program is designed to meet the following goals:

 

·               The activities are specifically tailored to meet the needs of each participant by building on their strengths and abilities. The main goal of each activity is for the participants to feel successful at that activity and, thus, increase their self-esteem.

·               Our activities program aims at fostering the participant’s maximum level of independence. Each participant is encouraged to do as much as they can on their own; help and guidance is provided to those who need it, and only to the extent necessary for success.

·               Another goal of our activities program is to help the participants achieve or maintain their optimal level of functioning, both physically and mentally. To achieve this, the activities are divided into two major groups: physical and mental.

·               Activities like stretching exercises, bowling, bat-the-balloon, dart ball, horseshoe throw, basketball etc. help keep them physically active while having fun. All these activities are modified so that almost all the participants can participate in them and feel successful.

·               The above activities improve circulation, joint flexibility and muscle strength. Improved muscle strength and balance helps the participants with basic activities of daily living (ADL). Strong ADLs are crucial to safely maintaining a degree of independence and, in turn, self-esteem.

·               Number games like Bingo, Dominos, High or Low pricing games, etc. help keep participants in touch with numbers and basic math.

·               Wheel of fortune, The Alphabet game, Word searches, Opposites, Rhyming words etc. help participants maintain language skills.

·               Current events and “Today in History” help participants with reality orientation. Current events are reviewed in relation to the things that happened in the past. This also jogs their memory and provides an opportunity for reminiscence.

·               Music is something that almost every participant has been able to relate to. Singing old, familiar and popular songs as well as hymns not only jogs the memory but also seems to provide an emotional outlet and enjoyment.

·               Most of the participants love to do small jigsaw puzzles. It encourages them to think abstractly and also provides an opportunity for problem solving.

·               Simple arts and crafts projects give them an opportunity to think at an abstract level and express their creativity.

·               Every one wants to feel needed and to help others. Our participants help the local chapter of the Alzheimer’s Association by collating and addressing the newsletters. They help in the same way with the quarterly newsletters of TCC. This encourages self-esteem through helping others.

·               Celebrating each participant’s birthday individually with a birthday party lets each participant know they are special and loved.

·               The group setting of our program gives ample opportunities for the participants to socialize and make new friends who really care about one another. This reduces the social isolation that many face.

At Trinity Care Center, Volunteers are a vital part of our program.  Our volunteers believe in, “Sharing is Caring.”  They enrich our program with their special talents, skills, dedication, and patience along with their loving and caring attitude. Families of our participants, individuals from our community, and various groups within our community have consistently and generously shared their time, talents, and resources in many different ways:

 

·      Assisting our staff during different activities.

·      Visiting with the participants.

·      Reading to the participants.

·      Sharing talents such as music, story telling, and crafts.

·      Playing games with the group or individuals.

·      Helping in maintaining the building and the yard.

·      Serving on the volunteer board of directors.

·      Serving on the volunteer advisory board.

·      Helping with the planning and carrying out of fundraisers.

·      Writing grant proposals.

·      Hours of Operation:  TCC is open Monday through Friday from 8:00 AM to 5:00 PM.  Participants must be picked up by the 5:00 PM closing time.

·      Fees:  Fees are as follows:

$  7 – One Hour

$25 – Half Day (Four Hours)

$35 – Full Day (Eight Hours)

$155 – Full Week (Monday through Friday all day)

** A sliding scale is available for those in need of reduced rates.

·      Payment: Payment for the week should be made at the end of that week. Families planning on paying on a monthly basis must notify the Director. 

·      Scheduling: Most of our participants have designated hours and days of the week for attending.  The family may make changes or cancellations up to the day before service.  This will allow for proper staffing to be arranged.

·      Holidays:  Holiday closures are as follows:  New Year’s Day, Good Friday, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and the Friday following, Christmas Day, and the week between Christmas and New Year’s Day.

·      Inclement Weather: When the Tullahoma City Schools are closed due to weather conditions, TCC will also be closed (e.g. snow days).  No fee will be charged for these days.  To clarify:  If Tullahoma schools remain open, although other schools in the surrounding area are closed, TCC will remain open.

·      Safety Escort:  Each participant needs to be escorted inside TCC by a caregiver and the staff made aware of the turnover of responsibility.
